School & Work Accommodation Forms

If you need a school or work accommodation form, you have the option to submit it for review through the "clinical messages" feature on your Frida portal. Following this submission, you'll need to attend a follow-up appointment with your clinician to finalize the form.

Please be aware that accommodation forms are handled on a case-by-case basis, and we cannot guarantee their completion or guarantee their acceptance by your educational institution or workplace.

If you decide to share a form with your clinician, kindly ensure the following steps are taken:

• After acquiring the form from your institution, please complete it thoroughly and provide your practitioner with all the essential details regarding your required accommodations. Upon receiving the form, our care team will schedule a follow-up appointment with your clinician to finalize the form.                                                                                                                             *Please note that there is a $79 fee for these follow-up appointments.

In situations where it's determined that your form cannot be completed, we can offer a general diagnosis letter or a standardized letter of support as an alternative.
