Please be aware that accommodation forms are handled on a case-by-case basis, and we cannot guarantee their completion or guarantee their acceptance by your educational institution or workplace.
If you decide to share a form with your clinician, kindly ensure the following steps are taken:
- After acquiring the form from your institution, please complete it thoroughly and provide your practitioner with all the essential details regarding your required accommodations.
- Upon receiving the form, our care team will schedule a follow-up appointment with your clinician to finalize the form. *Please note that there is a $99 fee for these follow-up appointments.
If it's determined that your form cannot be completed, we can offer a general diagnosis letter as an alternative.
