Learn more about completing forms here: Completing medical questionnaires before your initial assessment & follow-up appointments
At your appointment, you'll discuss how you're feeling, and if needed, adjustments will be made to your medication dosage (titration). Your clinician will also determine the cadence of your appointments moving forward. In general, during the titration phase, patients meet with their clinician every 4-8 weeks and are issued prescriptions to cover the length of time between those appointments.
For the best results, it's crucial to re-complete your assessment forms and attend follow-up appointments about every 4 to 8 weeks as needed until you attain your optimal dose. Your clinician will thoroughly reassess your progress and well-being at each follow-up appointment.
The time it takes to achieve medication stability varies from person to person. Patients who have reached an optimal dose with their treatment plan typically require one appointment every 3 to 6 months.
