If you’re moving or have already moved, please let us know by sending us a message to confirm your new address, move date, and whether it's a temporary or permanent change.
Please note the following:
- If you are changing provinces, a new clinician will be assigned to you and they will take over your care. Once you are assigned a new clinician, you can book a follow up appointment with them so you can pick up where you’ve left off.
- If your new address also means a change in your preferred pharmacy, please forward your new pharmacy information to the Frida Care Team. Be sure to include the new pharmacy's name, address, phone, and fax number, so they can update your records.
